* In User.ini, change the following lines to:

That gave me 100% functionality. All buttons may be assigned right in UT2004, including the D-Pad directions. I like to set the D-Pad to strafe just like the stick, since it's easier for combos and dodging.
Note that you may need to calibrate the sticks at first by pushing them to all 4 corners, or they may spin wildly.
Hopefully this will work for others. It did for me.
On PC, supposedly, different gamepads--even different Logitech gamepads--need different settings. So the settings I've listed might not work with other models. But if you want to try different settings, I would start with moving the JoyZ and JoyR parameters into the other slots below (U, V, Sliders).
DISCLAIMER: I HATE gamepads for first-person shooters. I only bought one to practice up for games on other peoples' consoles. Now I can practice with both Halo and UT04. Plus it will give me a handicap when challenging friends who aren't gamers.
